We accept Medicare, Medicaid, Medicaid pending, Hospice, and self pay.

We accept Medicare and take assignment, which means we directly bill Medicare.  Patient are only billed, when there is an unpaid balance which is not covered by Medicare.   Medicare reimburses 80% of our professional fees (which are set by Medicare) after patients have met their annual deductible.  The 20% co-pay and annual deductible amount are due and payable at the time of service.

Fee for Service

Wound & Primary Care LLC does not participate in any insurance plans, except for Medicare.   For patients with insurance other than Medicare, our practice operates on a fee-for-service basis or membership fee.  That means we ask patients not covered by Medicare or are not  members of  Wound & Primary Care LLC to pay at time of service.  For those without insurance and our members we provide low cost labs and radiographic imaging studies.

Health Insurance Plan

If you do have a PPO-style health insurance plan, you can still elect to use your insurance elsewhere (specialists, pharmacy, etc.) just as normal when needed.  If you have questions about how health insurance is best structured with our services, we are happy to discuss with you.

Membership

We provide a membership at a fee, which is not an insurance policy.  Membership serves as an enhancement, as we provide for most of your primary care needs, and insurance provides for catastrophic coverage.  We are an independent, alternative way to receive your wound care, weight management and primary health care, but not a complete substitute for health “insurance”.  We encourage, but don't require, our members to carry some type of health insurance policy in case of unexpected, expensive health events such as surgery or hospitalization.

Our simple, monthly membership fee covers the vast majority of wound and primary care services without insurance hassles.  We help you navigate beyond unnecessary testing, treatments, and prescriptions, guiding you toward a healthy lifestyle.  Our goal is to prioritize your medical freedom, by giving you honest and unbiased care.  We do not allow the insurance company to dictate your health care needs, but work with you to make these important life decisions.

Membership fees are:

Our monthly fee in certain cases are lower than co-pay and the yearly fee is lower the majority of insurance deductibles.  Our membership fee(s) are started upon enrollment.  Future membership fees are paid by the calendar month on an ongoing basis until cancellation.  Payment of membership fees is required to remain an active member and receive service.  Two months in advance required, as we do not charge an initial enrollment fee.

Kids (18 & under)        $45/mo   or  $450 one time yearly payment
Adults (19-  69)            $55/mo   or  $550 one time yearly payment
Seniors (70+)               $65/mo   or  $650 one time yearly payment
Family                          $120/mo     Account may include up to 2 adults (parents) & their children (dependents age 25 & under).

ALL members must enroll for Auto-Pay of invoices with a Credit or pin-less Debit Card.  Due to our low staff and low overhead business model we cannot invoice or bill membership or other fees.  Auto-pay dates of 1st, 5th, 10th, 15th, 20th, 25th of month may be chosen.

Memberships does not require LONG-TERM CONTRACTS or commitments.  However, cancellation and re-enrollment may require a fee as noted below.

Cancellation

You may cancel your membership at anytime for any reason by phone, message or mail.  All charges, including membership fees, will continue to accrue until notice of cancellation is given.

Re-Enrollment

If a member discontinues membership - by choice or lack of payment -  the member may rejoin Wound & Primary Care LLC in the future, but will be required to pay an “Re-Enrollment” fee of $175 per member in addition to re-establish your membership(s) as active.

Service Fees

All services not covered by membership fees (e.g. supply cost for procedures, non-covered labs, etc.) may be required to be PAID IN-FULL AT THE TIME OF SERVICE.  Some charges may be added to the next month's invoice at our discretion. â€‹ These addition fees include wound debridement, casting, joint injections, abscess drainage, mole removal/skin, biopsy, and grown toenail.  Labor is covered in your membership fees.  Labs tests are provided at a minimal fee to members; and some are free, including basic urine tests, strep tests, and EKGs.  Labs not covered by membership fee are provided at wholesale, discounted price.

Imaging studies are provided by Radiology Assist at the time of this website publication prices were as listed below,

​Labs are referred to LaboratoryAssist providers of low cost labs.

Patient with insurance in some cases have their labs and imaging studies covered and my choose

Self pay

Initial visit start lasting up to 30 minute cost $165 and $25 for each additional 15 minutes.  Any required labs, imaging studies, and supplies procedure will be added to the cost..

 

Weight Loss Clinic

For non-members the initial evaluation cost $165 while follow up $40 for no intervention and if medication is provided in the form of an injection, an addition cost of medication is added.  A compounded version of the newer weight lose medication will be available at a markedly reduced cost.​  For members only the cost of the medication will be charged.​

Cancellation

All cancellations for scheduled visits must be done a minimum of 6 hours prior to scheduled time.  If failure to do so and this becomes a continuous problem a fee of $25 will be charged for each missed visit.
